About the Role

We are seeking a Senior Patient Care Specialist to join our team as a Dialysis Care Professional in a Skilled Nursing Facility (SNF) setting. As a key member of our multidisciplinary team, you will be responsible for providing comprehensive patient care to ensure the best possible outcomes. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

The ideal candidate will have significant experience in dialysis care, preferably in a SNF setting, and possess a strong understanding of patient-centered care principles. You will work closely with dialysis RNs and other healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient. The position involves flexible scheduling to accommodate patient needs, with an average 12-hour workday, but can vary in any combination to achieve a minimum 30-hour commitment for full-time employment.

Key Responsibilities:
  • Provide direct one-on-one patient care to deliver safe and effective dialysis treatment.
  • Collaborate with dialysis RNs to develop and implement comprehensive care plans that meet individual patient needs.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless delivery of care.

Salary Range

The estimated annual salary for this role is $50,000-$70,000, depending on qualifications and experience. In addition to a competitive salary, we offer a total rewards package that includes a 401(k) match, health insurance, and various other benefits.
