We are seeking a dedicated Patient Care Advocate to join our team at Upward Health. As a Patient Care Advocate, you will play a crucial role in delivering chronic care management to high complexity patients in their homes and communities.

Key Responsibilities:
  • Conduct direct outreach to patients via phone, in-person, mailings, and other strategies to enroll them into our program and collect key data about their needs.
  • Meet with patients in their homes or in the community to conduct needs assessments, set health goals, and facilitate in-home provider appointments.
  • Cultivate strong relationships with patients and primary care providers to ensure seamless continuity of care.
  • Utilize motivational interviewing techniques to engage with patients and support their self-management and shared decision-making.

This is a field-based position that requires a self-starter who can work independently in an unstructured environment. You will be responsible for managing your own schedule, prioritizing tasks, and making unscheduled home visits as needed.

Requirements:
  • At least 2 years of experience in a related field, such as Community Health Worker, Peer Support Specialist, or Medical Assistant.
  • High school graduate or GED required.
  • Valid driver's license and auto liability insurance.
  • Reliable transportation and ability to travel within assigned territory.
  • Technologically savvy with basic computer skills, including typing.
