Role Summary
The Clinical Pharmacy Leader will excel in delivering outstanding service to patients and collaborating with the interdisciplinary team, while also serving as a mentor and clinical guide for pharmacy students, residents, and staff. This role involves active participation in departmental initiatives and clinical development projects, including precepting pharmacy students during their practice rotations and supporting the Residency Program Director as a primary preceptor for PGY-1 residents. The Clinical Pharmacy Leader will engage in patient rounds, fostering relationships with the healthcare team, developing medication therapy plans, and contributing to the advancement of clinical services.
Key Responsibilities
- Oversee non-pharmacist staff by delegating tasks, verifying all orders, ensuring smooth workflow, and checking IVs and other medications prepared by technicians.
- Evaluate the necessity for non-formulary medications ordered in accordance with departmental guidelines, and communicate with prescribing physicians for alternatives.
- Confirm physicians' orders for accuracy in dosage, regimen, allergies, potential drug interactions, adverse reactions, and monitor laboratory data prior to medication dispensing. Utilize appropriate communication channels to inform healthcare providers of any concerns.
- Ensure all narcotic orders are documented, maintain an accurate inventory log, conduct regular controlled substance inventories, and ensure compliance with documentation protocols.
- Provide expert advice to healthcare professionals regarding drug interactions, incompatibilities, side effects, and appropriate medication administration.
- Actively engage in departmental programs and initiatives.
- Pharm D degree with completion of a PGY-1 Pharmacy residency and a minimum of 3 years of relevant experience. Alternatively, 5 years of post-graduate inpatient hospital experience in a clinical environment if no residency training has been completed. Board Certification or the intention to obtain it within two years is preferred.
- Maintain Pharmacist licensure as mandated by the Commonwealth of Massachusetts.
- Demonstrated ability to counsel and educate patients and their families, effectively communicate with physicians and healthcare professionals, and oversee the work of other staff members.
- Competency in providing direct patient care across all age groups, including neonates, pediatrics, adolescents, adults, and geriatrics.
- Full-time salaried position at 40 hours per week.
- Primarily day shifts with occasional evening rotations, every third weekend, and rotating holidays.
