Job Summary

We are seeking a highly skilled Clinical Staff Pharmacist to join our team at Trinity Health. This is a per diem position that requires coverage for evenings and overnights, with hourly shift bonus rates applying ($5-$10/hr).

Key Responsibilities
  • Provide excellent customer service to patients and other members of the interdisciplinary team.
  • Serve as a clinical leader to students, residents, and other staff members, actively participating in department programs and clinical/professional development projects.
  • Act as a preceptor and mentor to pharmacy students on Introductory Pharmacy Practice Experience (IPPE) and Advanced Pharmacy Practice experience (APPE) rotations, primary preceptor to PGY-1 residents, assisting the Residency Program Director and serving on the Residency Advisory Committee.
  • Round on patients and develop a relationship with the interdisciplinary health care team, providing medication therapy plans, monitoring patients, and contributing to the creation/implementation of clinical services.
  • Supervise non-pharmacist personnel by assigning duties, checking all orders, maintaining efficient workflow, and checking IVs and other medications prepared by technicians.
  • Assess the need for non-formulary items ordered according to department policy, and initiate contact with prescribing physicians for substitutes.
  • Verify physicians' orders for proper dose, regimen, allergies, potential drug-drug interactions/incompatibilities, adverse drug reactions, and monitor lab data on all patients before dispensing medication.
  • Utilize appropriate communication logs to notify physicians/nurses of potential problems.
  • Verify all narcotic orders, maintain perpetual inventory logs, perform weekly C-II inventory, and monthly controlled drug inventory, ensuring all forms are completed and filed appropriately.
  • Advise other healthcare professionals on drug interactions, incompatibilities, side effects, and proper administration of medications.
  • Actively participate in department programs and projects.
Requirements
  • PharmD degree and completion of a PGY-1 Pharmacy residency plus 3 years of relevant experience.
  • If no residency training, 5 years of post-graduate inpatient hospital experience in a clinical setting with experience in decentralized services.
  • Board Certification or acquisition of Board Certification in the next 2 years is desired.
  • Maintains Pharmacist licensure as required by the Commonwealth of Massachusetts.
