Propósito del Puesto
El Especialista Clínico en Farmacia proporcionará un servicio al cliente excepcional a los pacientes y a otros miembros del equipo interdisciplinario, actuando como un líder clínico para estudiantes, residentes y otros profesionales. Participará activamente en programas departamentales y proyectos de desarrollo clínico/profesional, actuando como preceptor y mentor para estudiantes de farmacia en las rotaciones de Experiencia de Práctica Farmacéutica Introductoria (IPPE) y Experiencia de Práctica Farmacéutica Avanzada (APPE), así como preceptor principal para residentes de PGY-1, asistiendo al Director del Programa de Residencia y formando parte del Comité Asesor de Residencia. Se espera que realice rondas activas con los pacientes y desarrolle relaciones con el equipo de atención médica interdisciplinario, proporcionando planes de terapia medicamentosa, monitoreando a los pacientes y contribuyendo a la creación/implementación de servicios clínicos.
Responsabilidades Clave
- Supervisar al personal no farmacéutico asignando tareas, revisando todas las órdenes, manteniendo un flujo de trabajo eficiente y verificando IVs y otros medicamentos preparados por técnicos.
- Evaluar la necesidad de artículos no formulados solicitados de acuerdo con la política del departamento, e iniciar contacto con el médico prescriptor para alternativas.
- Verificar las órdenes de los médicos para dosis adecuadas, regímenes, alergias, posibles interacciones/incompatibilidades de medicamentos, reacciones adversas y monitorear datos de laboratorio de todos los pacientes antes de dispensar medicamentos. Utilizar un registro de comunicación apropiado para notificar al médico/enfermero sobre problemas potenciales.
- Verificar que todas las órdenes de narcóticos sean recibidas, mantener un registro de inventario perpetuo, realizar inventarios semanales de C-II y mensuales de medicamentos controlados, asegurando que todos los formularios se completen y archiven adecuadamente.
- Asesorar a otros profesionales de la salud sobre interacciones de medicamentos, incompatibilidades, efectos secundarios y administración adecuada de medicamentos.
- Participar activamente en programas y proyectos del departamento.
- Título de Pharm D y finalización de una residencia de farmacia PGY-1 más 3 años de experiencia relevante. Si no se cuenta con formación de residencia, se requieren 5 años de experiencia en un entorno clínico hospitalario.
- Licencia de Farmacéutico en Massachusetts.
- Certificación de la Junta o adquisición de la Certificación de la Junta en los próximos 2 años es deseada.
- Salario a tiempo completo de 40 horas por semana.
