About the Role

We are seeking a compassionate and skilled Registered Nurse to join our team as a Per Diem RN Case Manager. As a key member of our Home Health team, you will provide high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Administer skilled nursing care to patients in their homes, teaching and supervising family members and other caregivers as needed.
  • Act as a liaison between physicians, patients, and the home health team, ensuring seamless communication and care coordination.
  • Provide direct patient care services, including evaluating patient needs and developing individualized plans of care.
  • Manage and collaborate with interdisciplinary teams to ensure comprehensive patient care.
  • Maintain accurate and up-to-date patient clinical records, adhering to regulatory and company standards.
  • Participate in quality assessment and performance improvement initiatives, contributing to the growth and development of our team.
Requirements
  • RN license in the state of employment (or eligibility to obtain)
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
  • Additional experience in hospital settings, such as Staff RN, Med Surg, PACU, Float pool, ER, or critical care, is highly valued
  • Current driver's license and automobile insurance
  • Current CPR certification
