Compensation and Benefits

As a Home Health Nurse Practitioner with Matrix Medical Network, you can expect a competitive compensation package, including a base pay rate of $90.00 or $95.00 per completed assessment, with opportunities for flexible scheduling and mileage reimbursement.

Our PRN providers are non-benefited, except for limited voluntary benefits. However, with a commitment of 25 hours or more per month, you can enjoy a range of perks, including no show payments for 10 or more booked assessments per month.

Job Description

As a Home Health Nurse Practitioner, you will conduct adult and geriatric assessments, review medical histories, and develop personalized care plans to improve the health and well-being of our patients. You will work collaboratively with physicians, case managers, and other healthcare professionals to ensure seamless care transitions and exceptional patient outcomes.

Our Comprehensive Health Assessment provides unmatched insight into a patient's overall health and well-being, allowing us to identify potential care gaps and develop targeted interventions. With a nationwide network of advanced practice providers, you will have the opportunity to build and maintain relationships with colleagues and patients across the country.

Required Skills and Qualifications
  • Master's degree in nursing or a related field
  • Current RN and NP licensure in the state of practice, with prescription authority or the ability to obtain it
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Excellent verbal and written communication skills
  • Strong computer skills and familiarity with electronic health records

Travel Requirements

  • Valid state driver's license and reliable transportation
  • Ability to travel, with occasional overnight stays
