Position Overview:

The Patient Care Coordinator plays a vital role within the Patient Care Coordination Team, dedicated to enhancing the health outcomes of our patients. This position involves utilizing reports from third-party payers alongside internally generated data to perform patient outreach.

In this role, the coordinator will assist patients in scheduling appointments, ensuring they receive proper follow-up instructions, and connecting them with necessary support service agencies. The Patient Care Coordinator will actively engage with physician practices to promote continuous quality improvement initiatives, leveraging nationally recognized metrics such as HEDIS and evidence-based guidelines.

Key Responsibilities:

  • Facilitate communication and provide support to primary care providers regarding HEDIS, health maintenance, and chronic condition management for their patient panel.
  • Work under general supervision, adapting to various shifts and potentially exceeding 40 hours per week as needed.
  • Remain available on campus during severe weather conditions or emergencies.

Qualifications:

  • Completion of an accredited medical assisting program or graduation from a formal medical services training program in the United States Armed Forces, or a Practical Nursing Degree.
  • A minimum of two years of experience in a clinical or healthcare environment.
  • If licensed as an LPN, a current Florida LPN License is required. Medical Assistants must hold certification from recognized organizations such as AAMA, AMT, NCMA, or others, along with BLS certification from the American Heart Association.
  • Demonstrated skills in physician relations, consensus-building, independent work, problem-solving, and clinical expertise.
  • Proficiency in data-driven decision-making, excellent communication abilities, and familiarity with software applications such as Word, Excel, and PowerPoint, as well as EHR navigation.
  • Knowledge of medical terminology, anatomy, and physiology is essential.
