Clinical Pharmacist

Job Summary
We are seeking a highly skilled Clinical Pharmacist to join our team in the Centralized Refill Program and Hypertension Remote Management Program. As a key member of our pharmacy team, you will be responsible for supporting prescription refill renewal authorization for MaineHealth Medical Group patients, utilizing established institutional guidelines and protocols.
Key Responsibilities
- Enhance patient outcomes through appropriate medication management, utilizing evidence-based medicine principles.
- Triage additional information to facilitate timely refill requests for primary care clinic patients.
- Foster interdisciplinary collaboration with providers, pharmacists, and medical assistants to optimize medication therapy.
Requirements
- Bachelor of Science in Pharmacy Degree from a recognized/accredited School/College of Pharmacy required.
- Doctor of Pharmacy (PharmD) Degree or Master's Degree in Hospital Pharmacy or Business Administration preferred.
- Current applicable state(s) license with Board of Pharmacy required.
- Three years of experience in compounding preferred in a large academic medical center preferred.
