Key Responsibilities- Assess and evaluate member needs and requirements to develop personalized care plans
- Coordinate services and resources to ensure members receive the necessary support to maintain optimal well-being
- Collaborate with interdisciplinary teams, including healthcare providers, social workers, and community resources, to ensure comprehensive care
- Provide education, motivation, and encouragement to members to promote healthy behaviors and lifestyle choices
- Monitor and evaluate care plans to ensure members are receiving the necessary services and support
- Bachelor's Degree in a related field, such as Health Services, Social Work, or Psychology
- At least 2 years of experience in a healthcare or social services setting
- Intermediate to advanced computer skills and experience with Microsoft Word, Excel, and Outlook
- AHCA Fingerprint Required
- Ability to work Monday-Friday, 8:30am-5:00pm
- Bilingual English/Spanish with ability to pass a Language Proficiency Exam
- Community Health Worker certification
- Prior nursing home diversion or long-term care case management experience
- Experience with electronic case note documentation and documenting in multiple computer applications/systems
- Experience working with geriatric population
- Experience with health promotion, coaching, and wellness
- Knowledge of community health and social service agencies and additional community resources
