Job Summary

We are seeking a highly skilled and compassionate Case Manager/Care Coordination Specialist to join our team at Boys Town. As a key member of our care coordination team, you will provide intensive and professional case management services to youth and families, utilizing a strength-based and comprehensive model.

Key Responsibilities
  • Provide case management services for Care Coordination Services, including assessment, service planning, and monitoring.
  • Develop and maintain effective working relationships with families, community resources, and service providers.
  • Identify and link youth to necessary services, resources, and supports, and monitor their progress.
  • Act as a youth and family advocate, empowering them to advocate for themselves in the community.
  • Assist youth and family members in navigating through various systems, including child welfare, juvenile justice, and healthcare.
  • Provide support in the development and utilization of functional skills to youth and family members.
  • Develop, monitor, and review service plans, and develop subsequent plans as needed to facilitate progress.
  • Maintain appropriate level of direct contact with youth and family members, and remain available for crisis intervention as needed.
  • Creates and manages discharge planning, including step-down placements, reunification, adoption, or transition plans.
Requirements
  • Bachelor's degree in Human Services or related field required.
  • Minimum 1 year of experience working with children and families required.
  • Possess a valid driver's license with a good driving record required.
  • Ability to provide own transportation to complete travel requirements of the job required.
  • Meet auto insurance requirements established by Boys Town policy and/or State and Local laws and pass an annual Motor Vehicle Registration (MVR) check required.
  • Available rotationally to provide crisis response and must be available to work non-traditional business hours including early mornings, nights, weekends, and holidays required.
Preferred Qualifications
  • Experience working with children and families in community-based programs or Boys Town programs preferred.
  • Bilingual may be preferred.
