This role is part of a new 30-day care transition program designed to reduce avoidable readmissions and improve patient care and well-being. As a key member of the team, you will work closely with our interdisciplinary team to provide support, follow-up, and access to resources for patients in the community.


Responsibilities include:

  • Designing and implementing operational details of the intervention
  • Following discharge plans and providing early engagement, patient education, and coaching
  • Assisting patients in overcoming potential barriers to care
  • Providing thorough ongoing patient follow-up

Position Qualifications

  • Current New York State RN License or Social Work License (LMSW or LCSW)
  • Degree in Nursing, Social Work or a related field
  • 3-5 years experience as a staff nurse, social worker or care coordinator
  • Prior experience in case management
  • Demonstrated commitment to interdisciplinary care management approach
  • Familiarity with government regulations for managed care plans
  • Excellent critical thinking, problem-solving and analytical skills
  • Excellent oral and written communication skills
  • Fully vaccinated against COVID-19
