Job Summary:

We are seeking a highly skilled and compassionate Licensed Clinical Social Worker to join our team at our A31 Sidney Baer (NYC) location. As a key member of our interdisciplinary team, you will be responsible for coordinating and overseeing all aspects of psychosocial programming, ensuring that admission and discharge procedures are consistent with individual needs.

Key Responsibilities:
  • Plan, develop, and evaluate the psychosocial needs and concrete needs of program clients
  • Coordinate and oversee all aspects of psychosocial programming and services, including support groups for caregivers and participants
  • Ensure that staff who have case management responsibilities document case notes as required
  • Participate in the interdisciplinary team care planning process
  • Carry a reduced case management caseload, as necessary
  • Lead or co-lead psychosocial groups, including support groups for family members, significant others, and caregivers
  • Provide psychotherapy for a select caseload
  • Perform interagency liaison work with other service agencies, as needed
  • Maintain client case notes and statistical records; ensure documentation of progress notes
  • Participate in Quality Assurance program to evaluate, monitor, identify problems, and institute revisions to maintain quality of client care and provide services in accordance with current standards of professional practice
  • Attend and participate in seminars and continuing education programs to expand knowledge of current trends in the field of specialty and in issues concerns of the HIV population
Requirements:
  • Master's degree in Social Work
  • Minimum Related Work Experience: 2-4 years
  • NYS Licensed LMSW, LCSW, or LMHC
Salary Range:

$65,000 - $100,000 annual, depending on licensure and experience
