About the Role

We are seeking a dedicated Per Diem Hospice Registered Nurse (RN) to join our team at Interim HealthCare Inc in Lexington, South Carolina. As a key member of our interdisciplinary group, you will provide high-quality care and support to patients and their families during their final months, weeks, and days.

Job Description

Your primary responsibilities will include:

  • Completing comprehensive assessments of patients' physical status and psychosocial needs
  • Developing individualized plans of care in collaboration with the IDG, patient, and physician
  • Initiating ongoing assessments to ensure optimal symptom management and quality of life
Requirements

To be successful in this role, you must possess:

  • A graduate degree from an accredited Registered Nurse program
  • An active, unrestricted Registered Nurse license in South Carolina or compact multi-state license
  • Twelve months of accumulated experience as a registered nurse within the last five years
  • Prior hospice care experience is preferred but not mandatory
