Job Summary
The successful candidate will be responsible for assessing patients' needs, developing tailored treatment plans, and delivering high-quality rehabilitation services. A strong clinical expertise, excellent communication skills, and ability to work collaboratively with an interdisciplinary team are essential for this role.
Responsibilities
- Collects and records patient care data, including initial evaluations, daily notes, reassessments, progress reports, discharge plans, and communication with the team.
- Prepares for evaluations, selects and administers tests, and synthesizes results to formulate diagnoses, goals, and plans of care.
- Establishes treatment plans based on evaluation results, goals, and best practices, collaborating with the interdisciplinary team, patients, and their support systems.
- Identifies needs for consultations, referrals, and modifications to plans of care and goals.
- Provides education and training to patients and their support systems, focusing on ADLs, IADLs, therapeutic activities, rehabilitation equipment prescription and training, and community reintegration.
Requirements
To be considered for this role, you must have a Bachelor's or Master's degree in Physical Therapy, a valid licensure in the state where you practice, completed CEU requirements, and experience/knowledge with EMR software. Excellent oral and written communication skills, strong clinical decision-making abilities, and a commitment to maintaining patient confidentiality are also essential.
