Job Summary

We are seeking a skilled Home Health Physical Therapist to join our team at Alternate Solutions Health Network LLC. As a Home Health Physical Therapist, you will provide high-quality physical therapy services to patients in their homes, promoting independence and improving their overall health and well-being.

Key Responsibilities
  • Develop and implement individualized plans of care for patients, taking into account their unique needs and goals.
  • Provide skilled physical therapy services, including exercises, stretches, and other interventions, to help patients achieve optimal mobility and function.
  • Conduct thorough assessments and evaluations to identify patients' strengths and weaknesses, and develop strategies to address any deficits.
  • Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care and coordination.
  • Communicate effectively with patients and their families, providing education and support to promote understanding and adherence to treatment plans.
  • Document patient progress and outcomes, and maintain accurate and timely records.
Requirements
  • Current licensure as a Physical Therapist in the state of employment.
  • Valid driver's license and auto insurance.
  • Ability to lift, bend, and move patients as needed.
