Job Title: Home Health Physical Therapist

Alternate Solutions Health Network LLC is seeking a skilled Home Health Physical Therapist to join our team in Cuyahoga Falls, Ohio.

Job Summary:

We are looking for a compassionate and dedicated Physical Therapist to provide high-quality home-based therapy care to our patients. As a Home Health Physical Therapist, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that promote patient independence and well-being.

Responsibilities:
  • Provide skilled physical therapy services in patients' homes as ordered by the attending physician.
  • Conduct thorough assessments and develop plans of care that address patients' physical therapy needs.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care coordination.
  • Document all aspects of patient care, including assessments, treatments, and progress.
  • Participate in ongoing education and training to stay current with best practices in physical therapy.
Requirements:
  • Current licensure as a Physical Therapist in the state of employment.
  • Minimum one year of experience as a Physical Therapist in an acute setting.
  • Home care experience is desired.
  • Ability to effectively communicate with patients, families, and healthcare providers.
  • Knowledge of therapy best practices and ability to make appropriate physical therapy judgments.
