Job Title: Home Health Physical Therapist

Alternate Solutions Health Network LLC is seeking a skilled Home Health Physical Therapist to join our team in Cuyahoga Falls, Ohio.

Job Summary:

We are looking for a compassionate and detail-oriented Physical Therapist to provide high-quality care to patients in their homes. As a Home Health Physical Therapist, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that promote patient independence and improve quality of life.

Responsibilities:
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective treatment plans.
  • Implement and modify treatment plans as needed to ensure optimal patient outcomes.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Document all aspects of patient care, including assessments, treatment plans, and progress notes.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Current licensure as a Physical Therapist in the state of Ohio.
  • Minimum one year of experience as a Physical Therapist in an acute setting.
  • Home care experience preferred.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Valid driver's license and reliable transportation.
