Job Summary

We are seeking a skilled Home Health Physical Therapist to join our team at Alternate Solutions Health Network LLC. As a Home Health Physical Therapist, you will provide high-quality physical therapy services to patients in their homes, promoting independence and improving their overall health and well-being.

Key Responsibilities
  • Develop and implement individualized plans of care for patients, taking into account their unique needs and goals.
  • Provide skilled physical therapy services, including exercises, stretches, and other interventions, to help patients achieve optimal function and mobility.
  • Conduct thorough assessments and evaluations to identify patients' strengths and weaknesses, and develop strategies to address any deficits.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and optimal patient outcomes.
  • Collaborate with other healthcare professionals, such as occupational therapists and speech therapists, to provide comprehensive care and support.
  • Maintain accurate and detailed records of patient progress, including treatment plans, progress notes, and outcome measures.
  • Stay up-to-date with the latest research, best practices, and industry developments to ensure the highest level of care and expertise.
Requirements
  • Current licensure as a Physical Therapist in the state of employment.
  • Valid driver's license and auto insurance.
  • Ability to lift, bend, and move patients as needed.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
