Job Title: Case Manager

Pinnacle Services is seeking a skilled Case Manager to join our Hennepin County team. As a Case Manager, you will work with individuals with developmental disabilities, serious and persistent mental illness, chemical dependency issues, physical disabilities, or aging needs.

Job Responsibilities:
  • Assess the needs of individuals and develop personalized service plans.
  • Coordinate resources and services to support individuals in achieving their goals.
  • Monitor service delivery and work with providers to ensure individuals' needs are met.
  • Develop and implement service plans that promote person-centered care.
  • Attend regular meetings with individuals and their interdisciplinary teams.
  • Document client-related work to create billable units.
Requirements:
  • Graduate degree in social work, psychology, sociology, or a closely related field.
  • One year of experience as a social worker, case manager, or care coordinator in a public or private social service agency.
  • Valid driver's license and acceptable driving record.
  • Vehicle with current insurance.
  • Successful completion of local and/or state background checks.
Preferred Qualifications:
  • One year of experience in education or treatment of individuals with developmental disabilities or related conditions.
  • Minimum of one course focusing on developmental disabilities.
  • LSW or higher form of BOSW licensure.
