POSITION SUMMARY: The Critical Care Pharmacy Specialist is responsible for ensuring the delivery of safe, effective, and economical medication therapy for patients in critical care settings. This role involves active communication with healthcare professionals and patients regarding medical treatment and quality of life, with a strong emphasis on monitoring and documenting pharmacotherapy outcomes. The specialist will provide in-depth drug information to medical staff, engage in clinical functions, and ensure compliance with care standards for patients across various age groups.
KEY RESPONSIBILITIES:
- Engage in daily patient care discussions, evaluating medication therapies for appropriateness, dosage, and potential interactions. Document all medication adjustments in patient records.
- Maintain and update patient medication profiles, promoting formulary compliance and coordinating procurement of non-formulary medications as necessary.
- Demonstrate competency in managing medication therapies for diverse patient populations, including adults, pediatrics, geriatrics, and psychiatric patients.
- Review pharmacy records to ensure medication safety and efficacy, documenting any adverse drug reactions or medication errors.
- Conduct pharmacokinetic assessments, IV to PO conversions, and therapeutic interchanges efficiently and accurately.
- Participate in clinical pharmacy initiatives and respond to medication-related inquiries from healthcare professionals and patients.
- Act as a consultant for the Pharmacy & Therapeutics committee and contribute to performance improvement initiatives.
- Engage in public speaking and educational presentations to promote pharmacy services and enhance staff knowledge.
- Contribute to the development and implementation of pharmaceutical care programs aimed at optimizing patient outcomes.
- Ensure compliance with all regulatory standards and provide regular updates to the Clinical Pharmacy Manager regarding operations and projects.
- Respond to medical emergencies as required and provide patient counseling on discharge medications.
- Work rotating shifts as needed.
QUALIFICATIONS:
- Pharm.D. or equivalent from an accredited School of Pharmacy and a valid pharmacist license. Completion of a Critical Care residency or equivalent clinical experience is preferred.
- Thorough understanding of pharmacy laws and regulations, along with proficiency in medication management and technology.
- Strong verbal and written communication skills, with the ability to collaborate effectively within a multidisciplinary team.
- Ability to work independently, manage stress, and prioritize tasks in a dynamic environment.
