About the Role:
VHC Health Physicians is seeking a highly skilled Advanced Practice Provider (APP) to join our team. As an APP, you will play a critical role in delivering high-quality patient care and supporting the objectives of our organization.
Key Responsibilities:
- Collaborate with physicians to develop and implement patient care plans
- Provide primary and specialty care to patients in various settings
- Conduct thorough assessments, diagnose conditions, and develop treatment plans
- Order and interpret diagnostic tests, prescribe medications, and perform procedures as needed
- Document patient information accurately and maintain confidentiality
- Participate in quality improvement initiatives and contribute to the development of policies and procedures
Requirements:
- Current licensure as a Nurse Practitioner or Physician Assistant in the state of Virginia
- Minimum of two years of experience as an APP in a similar setting
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
