We are seeking a compassionate and skilled Hospice RN Case Manager to join our team at Interim HealthCare. As a Hospice RN Case Manager, you will play a vital role in delivering high-quality patient care and support to individuals and their families during the most challenging times.

Key Responsibilities:
  • Develop and implement comprehensive care plans that address patient needs and preferences
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication
  • Provide emotional support and guidance to patients and their families
  • Ensure accurate documentation and reporting of patient information
Requirements:
  • Associate degree in Nursing (ADN) or Associate of Applied Science in Nursing (AAS) required; Bachelor of Science in Nursing (BSN) preferred
  • Current license to practice as a Registered Nurse (RN) in the state associated with this position
  • Current CPR/FA certification
  • Reliable transportation to/from care sites and/or work locations
  • One (1) year of professional experience practicing as a Registered Nurse (RN) in a hospice or similar setting preferred
