Comprehensive Case Manager

This position provides comprehensive care coordination for patients across the healthcare continuum, ensuring optimal clinical, financial, operational, and satisfaction outcomes. As a leader in process improvement activities, the Comprehensive Case Manager collaborates with healthcare teams to develop and manage interdisciplinary patient discharge plans, effectively communicating these plans throughout the care continuum. Additionally, this role involves evaluating medical necessity and appropriateness of care, optimizing patient outcomes, and identifying issues that may delay patient discharge, facilitating resolution of these issues. The Comprehensive Case Manager works closely with physicians, payers, and other members of the healthcare team to collect and communicate pertinent information, fulfilling utilization and regulatory requirements.
The estimated salary for this position is $90,000 - $120,000 per year, depending on experience.
