Comprehensive Nutrition Care Manager

Job Overview: Foundation Health Services seeks an experienced Nutrition Specialist to manage a complex caseload of patients and provide comprehensive nutritional assessments, develop care plans, and monitor outcomes. This role requires strong communication skills, attention to detail, and the ability to work effectively in an interdisciplinary team environment.
Responsibilities:- Provide comprehensive nutrition assessments and develop individualized care plans for patients.
- Evaluate the effectiveness of care plans and communicate with the healthcare team to ensure optimal nutrition outcomes.
- Design and implement effective nutrition education programs for patients, families, and healthcare professionals.
- Collaborate with peers to develop Nutrition Standards of Practice and Education materials.
- Prioritize continuous quality improvement activities to enhance nutrition care outcomes.
- Bachelor's degree in Nutrition and/or Dietetics.
- Registered Dietitian (RD) or RD Eligible credentials within 6 months of hire.
- Maintain ongoing registration through the Commission on Dietetic Registration of the American Dietetic Association.
The estimated salary range for this position is $85,000 - $110,000 per year.
