Key Responsibilities

The Manager of Case Management will oversee the development and implementation of a comprehensive Case Management Program. This role includes:

  • Providing leadership and guidance to case management staff, including social workers and discharge planners.
  • Assessing patient needs and designing services that align with our hospital's mission.
  • Coordinating services to enhance quality and efficiency using continuous improvement methodologies.
  • Managing the quality and resource allocation for all patients admitted to our facilities.
  • Conducting thorough reviews of medical records to ensure compliance with clinical standards.

Qualifications

Required:

  • Current licensure as a clinician in your state.
  • A minimum of five years of post-graduate experience in an accredited social work program for Licensed Clinical Social Workers.
  • At least five years of experience in a Case Management role.
  • Strong analytical skills for problem identification and data evaluation.
  • Proficiency in basic to intermediate computer applications.

Preferred:

  • Certification in Case Management or a related field.
  • Current Basic Life Support (BCLS) certification.
  • Familiarity with Milliman and InterQual Criteria.
