Key Responsibilities:
- Assess physician medication orders for suitability of drug selection, dosage, potential interactions, and administration routes. Provide recommendations for therapy adjustments to physicians and facility staff as necessary.
- Maintain a comprehensive patient/resident database that includes relevant medical history, clinical and laboratory data, and concurrent medications (including over-the-counter and home remedies).
- Ensure that necessary laboratory tests for monitoring high-risk conditions are ordered, assessed, and discussed with the healthcare team.
- Collaborate with the healthcare team to offer clinical and technical support as required.
- Document all communications with physicians, nurses, and other healthcare team members, as well as patients/residents and their families, in the medical record.
- Implement policies and procedures for the safe procurement, storage, distribution, and usage of medications and biologicals, including their disposal.
- Acquire and maintain all required documentation in compliance with federal and state regulations and organizational policies.
- Notify the Pharmacy Manager of any medication-related errors.
- Assist facility staff with procedures related to the safe and effective management of medications.
- Perform additional tasks as assigned.
Qualifications:
- Bachelor's Degree in Pharmacy; Pharm D preferred.
- Minimum one year of experience in long-term care settings.
Licenses/Certifications:
- As mandated by governmental regulations and business requirements.
- Detail-oriented, goal-driven, team-oriented, with strong oral communication skills.
