Job Title: Travel Nurse RN - Home Health Specialist
Job Summary:
SkyBridge Healthcare Home Health & Hospice is seeking a skilled Travel Nurse RN to join our team as a Home Health Specialist. As a Travel Nurse RN, you will provide high-quality patient care in a home health setting, working closely with our interdisciplinary team to deliver exceptional patient outcomes.
Responsibilities:
- Provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
- Collaborate with the healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies to address them.
- Administer medications and treatments as prescribed by physicians, ensuring accurate and timely delivery.
- Document patient progress and outcomes, maintaining accurate and detailed records of care provided.
- Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
- Current RN licensure in the state of employment
- Minimum 1 year of experience in home health or a related field
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
- Strong problem-solving and critical thinking skills
