We are seeking a compassionate and skilled Registered Nurse to join our team as a Case Manager in Hospice. Located in Danville, VA, Commonwealth Hospice offers a unique opportunity to make a meaningful difference in the lives of patients and their families.
About the RoleThis full-time position involves conducting home visits for patients requiring end-of-life care. As a Hospice RN, you will develop strong relationships with patients and their loved ones, providing empathetic support and guidance throughout their journey.
Key Responsibilities- Deliver high-quality patient care by implementing individualized plans of care, taking into account each patient's unique needs and circumstances.
- Evaluate personal and professional performance, identifying areas for improvement and implementing changes to enhance productivity and quality of care.
- Collaborate with the hospice team to ensure seamless delivery of services, promoting a culture of excellence and compassion.
- Maintain accurate records and communicate effectively with colleagues, patients, and families.
- Minimum one year of clinical experience as a Registered Nurse.
- Current licensure in good standing in the state of practice.
- CPR certification and valid driver's license with reliable transportation.
- Experience working with terminally ill patients is highly desirable.
