About the Role

This full-time position involves conducting home visits for patients requiring end-of-life care. As a Hospice RN, you will develop strong relationships with patients and their loved ones, providing empathetic support and guidance throughout their journey.

Key Responsibilities
  • Deliver high-quality patient care by implementing individualized plans of care, taking into account each patient's unique needs and circumstances.
  • Evaluate personal and professional performance, identifying areas for improvement and implementing changes to enhance productivity and quality of care.
  • Collaborate with the hospice team to ensure seamless delivery of services, promoting a culture of excellence and compassion.
  • Maintain accurate records and communicate effectively with colleagues, patients, and families.
Requirements
  • Minimum one year of clinical experience as a Registered Nurse.
  • Current licensure in good standing in the state of practice.
  • CPR certification and valid driver's license with reliable transportation.
Additional Requirements
  • Experience working with terminally ill patients is highly desirable.
