Responsibilities:

  • Deliver holistic care that addresses patients' physical, emotional, and spiritual needs
  • Develop and implement individualized Plans of Care that align with patient goals and preferences
  • Collaborate with healthcare providers, family members, and other stakeholders to ensure effective care coordination
  • Document patient interactions, progress notes, and treatment plans in accordance with regulatory requirements
