Advanced Practice Clinician, Integrated Care Specialist

We are seeking a highly skilled Advanced Practice Clinician to join our team at SCAN Health Plan. As a key member of our integrated care delivery team, you will provide primary care to our members, focusing on geriatric medicine and chronic disease management.
Key Responsibilities- Provide comprehensive primary care to assigned members, including physical exams, assessments, and chronic disease management.
- Collaborate with our case management team to ensure timely access to necessary services and coordinate care with specialists and community-based organizations.
- Engage with primary care physicians and other healthcare professionals to promote disease prevention, health promotion, and shared decision-making.
- Act as a resource for members and their families, providing education and support to promote healthy aging and independence.
- Participate in on-call programs and virtual health visits to ensure 24/7 access to care.
- Master's degree in Nurse Practitioner or Physician Assistant program, with education in adult/geriatric medicine preferred.
- Current California Nurse Practitioner license or Physician Assistant Certification (PA-C) in good standing.
- Knowledge of HCC and ICD-10 coding and documentation, as well as wound care and management.
- Ability to identify health care needs and refer and coordinate with other resources/programs appropriately.
- Strong interpersonal skills and change agent mindset to build relationships with healthcare teams and patients.
- Competitive salary range: $125,400.00 to $200,600.00 annually.
