Job Summary

Acentra Health is seeking a skilled Clinical Evaluator to join our team. As a Clinical Evaluator, you will conduct face-to-face assessments to facilitate appropriate placement and next level of care in a variety of settings.

Key Responsibilities
  • Conduct thorough assessments to evaluate the needs of patients, including reviewing medical records and interacting with healthcare professionals, patients, and their families.
  • Analyze and assess psychological, emotional, behavioral, and physical functioning to determine the level and scope of care required.
  • Develop informed level of care recommendations based on thorough clinical evaluations, ensuring compliance with contractual and regulatory standards.
  • Collaborate with healthcare professionals and other stakeholders to ensure effective care planning and coordination.
  • Utilize technology to efficiently manage and track assessment activities, including the state's electronic system and proprietary platforms.
  • Stay up-to-date on current regulations and research related to long-term care, mental illness, medical conditions, and geriatrics.
Requirements
  • Minimum 2-3 years of experience assessing individuals with mental or psychiatric disorders, including complex differential diagnoses.
  • Active unrestricted licensure in Washington in a healthcare or related field, such as a Licensed Psychologist, Psychiatric Nurse, Social Worker, Mental Health Counselor, Marriage and Family Therapist, or Social Worker Independent Clinical License.
  • Graduation from an accredited college with a relevant degree program.
  • Expertise in assessing and analyzing psychological, emotional, behavioral, and physical functioning.
  • Working knowledge of Preadmission Screening and Resident Review (PASRR) requirements per Federal Regulation 42 CFR, Chapter 483.
  • Knowledge of psychiatric disorders as defined by the most current Diagnostic and Statistical Manual in use in Washington.
  • Experience in determining a clinical diagnosis accounting for psychological and behavioral issues.
  • Knowledge of standards of practice related to the determination of Level of Care and standards of practice related to mental health, geriatrics, and other disorders commonly necessitating long-term care placement.
  • Experience with public mental health and medical programs, as well as public sector services and supports.
  • Proficient with Microsoft Office applications, including Excel, Word, PowerPoint, Teams, Outlook, and other web-based software.
  • Excellent communication skills for conveying information to individuals and groups through spoken, written, and electronic media.
  • Keen attention to detail, with the ability to prioritize and execute tasks efficiently and in a timely manner.
