Role Overview:

The Staff Pharmacist is responsible for compounding and dispensing medications as per physician directives. This role involves collaboration with medical personnel regarding potential drug interactions, patient medical histories, and the administration of specific medications. The pharmacist will address patient inquiries regarding medications and dosage guidelines, ensuring compliance with all necessary pharmacy documentation and regulatory standards.

Key Responsibilities:

  1. Assess the appropriateness of medication dosages based on the patient's diagnosis, weight, age, and body surface area.
  2. Identify and address issues related to drug therapy, including allergies, interactions, therapeutic duplications, and adverse reactions.
  3. Evaluate the suitability of selected medications, initiate therapy when necessary, and recommend cost-effective alternatives.
  4. Determine the necessity for non-formulary medications.
  5. Ensure patients receive prescribed medications accurately in terms of dosage, route, and regimen.
  6. Maintain high standards for medication quality and preparation, ensuring proper labeling and use of auxiliary comments.
  7. Efficiently process all medication orders, establishing priorities and timelines.
  8. Provide professional support in patient care settings, monitoring for optimal therapeutic outcomes and advising healthcare providers accordingly.
  9. Engage in the education and training of patients, pharmacy technicians, and students, contributing to discharge planning.
  10. Conduct drug information activities, accurately processing information requests and maintaining documentation.
  11. Order medications and supplies from local wholesalers and manage back orders with manufacturers for items in short supply.
  12. Ensure substitution items are procured when original medications are unavailable, with appropriate managerial oversight.
  13. Immediately inform the Pharmacy manager of any unavailable items at the time of purchase.
  14. Coordinate the loan/borrow process, manage recalls, and oversee annual departmental inventory.
  15. Perform additional duties as assigned.
