The Clinical Pharmacist is responsible for compounding and dispensing medications as per physician directives. Key Responsibilities:
- Assess the appropriateness of all dosages based on the patient's diagnosis, weight, age, and body surface area.
- Identify and address issues related to drug therapy, including allergies, drug interactions, therapeutic duplications, and adverse reactions.
- Verify the suitability of selected medications, initiate therapy when necessary, and recommend equally effective but more cost-efficient alternatives.
- Evaluate the necessity for all non-formulary medications.
- Ensure that patients receive the prescribed medications accurately regarding dosage, route, formulation, regimen, and generic substitutions.
- Guarantee that all medications meet the highest standards of quality and that all products are properly prepared and labeled, including appropriate auxiliary labels and comments.
- Efficiently process all orders, establishing priorities and ensuring timely processing.
- Act in a professional capacity within patient care areas, monitoring patients to ensure optimal therapeutic outcomes and making suitable recommendations to other healthcare providers.
- Participate in the education and training of patients, technicians, pharmacists, and students, serving as an educator and contributing to discharge planning.
- Conduct drug information activities, accurately processing all information requests and completing necessary documentation. Review patients for compliance with criteria regarding food-drug interactions and therapeutic drug monitoring, assisting in drug utilization review activities.
- Order medications and supplies from local wholesalers and manage back orders directly with manufacturers for items in short supply.
- Ensure that substitution items are procured when the original items are unavailable, with appropriate checks by management.
- Notify the Pharmacy manager immediately of any items that are unavailable at the time of purchase.
- Coordinate the loan/borrow process, manage recalls, and oversee annual departmental inventory.
- Perform additional duties as assigned.
