About the Role

We are seeking a skilled Registered Nurse to join our team as an Admission Nurse/Float Nurse. This part-time position involves providing coverage in the Portland metro area, working directly with patients and their families to deliver compassionate end-of-life care.

Job Description
  • Provide comprehensive nursing assessments and develop individualized plans of care for patients in their homes, adult foster homes, or residential/assisted living settings.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and timely interventions.
  • Maintain accurate and up-to-date patient records, including recording problems, plans, actions, and goals in accordance with established policies and procedures.
  • Deliver high-quality nursing care, attentive to the physical, emotional, and spiritual needs of patients and their loved ones.
Requirements
  • Current unencumbered Oregon licensure as a Registered Nurse is required prior to the first day of employment.
  • At least one year of professional nursing experience in hospice, palliative care, home health, medical-surgical care, or equivalent.
  • A valid driver's license, valid insurance, acceptable driving record, and access to a reliable automobile for travel to and from patient homes.
