Amergis Healthcare Staffing is in search of a Nurse Hospice Care Coordinator responsible for managing the continuum of care activities for designated patients, ensuring optimal resource utilization, service delivery, and adherence to medical protocols.


Key Responsibilities:
  • Conducts and oversees initial evaluations and ongoing assessments of patient conditions.
  • Records patient case details within a database system.
  • Performs monthly chart reviews and audits as necessary.
  • Engages in monthly case discussions, providing relevant information regarding patient needs and objectives.
  • Collaborates with the Program Director to develop and review individualized care coordination plans for patients.
  • Ensures that patients' medical requirements are met; consults with physicians as needed, coordinating treatment plans and advocating for patients when necessary.
  • Enhances understanding of medical factors impacting the target population.
  • Identifies and assists patients in accessing entitlements, resources, information, and referrals for psychosocial support.
  • Participates in Quality Assurance and Utilization Review activities as directed.
  • Empowers patients in making informed decisions regarding their care planning.
  • Maintains accurate and timely patient information, ensuring it is readily accessible for review and meets all regulatory requirements; assists in data collection for reporting and funding sources.
  • Fosters collaborative relationships within and between facilities to achieve goals; acts as a liaison among primary care providers, specialists, and patients.
  • Advocates for patients regarding service accessibility.
  • Adheres to state and national guidelines for nurse case management programs.
  • Engages in outreach initiatives to the entire target population as directed.
  • Recommends program or service modifications to address community service gaps.
  • Performs additional duties as assigned or necessary.

Qualifications:
  • Current RN license.
  • Minimum of one year of Case Management experience.
  • Current BLS certification as per contract requirements.
  • Current TB or Chest x-ray (if applicable).
  • Ability to effectively communicate information to and from relevant individuals (including supervisors, colleagues, and clients) through strong communication skills; proficiency in the English language may be required.
  • Proficiency in computer usage is required.
  • Must be at least 18 years of age.
