40 hours per week, Monday to Friday, with occasional evening shifts.
Role Summary:
The Clinical Nurse Specialist will deliver nursing care to patients within a healthcare setting.
Patient care encompasses various age groups, including:
- Pediatric (infants to 12 years)
- Adolescent (13 to 17 years)
- Adult (18 to 64 years)
- Geriatric (over 64 years)
Key Responsibilities:
Qualified candidates must possess the capability (with or without reasonable accommodation) to perform the following tasks:
- Provide clinical guidance via phone or electronic communication based on clinical assessments and triage protocols in line with regulatory standards.
- Consult with healthcare providers as needed.
- Deliver patient-specific education covering preventive care, chronic disease management, acute diagnosis, medications, and other relevant areas affecting patient outcomes.
- Prepare patients for consultations using established rooming standards.
- Administer injections and medications as prescribed by the healthcare provider, adhering to departmental protocols and regulatory standards.
- Assist healthcare providers with various procedures, minor surgeries, and examinations as directed.
- Conduct Point of Care Testing, including but not limited to hearing, flu, urine, and strep tests.
- Collect blood or other laboratory specimens and prepare them for further analysis.
- Authorize prescription refills as instructed by the healthcare provider.
- Assist with tracking tests and monitoring overdue results.
- Remove sutures and change dressings on wounds as necessary.
- Ensure examination rooms are well-stocked and sanitized after each patient visit.
- Maintain and clean medical instruments and equipment following each procedure.
- Support healthcare providers and patients in reviewing and updating care plans, assessing patient progress, and following up as necessary.
- Engage in proactive outreach for patients needing preventive and chronic disease services.
- Coordinate care activities among specialists and manage transitions of care to gather all necessary patient information.
- Assist in coordinating referrals, education, and community resources.
- Actively participate in initiatives aimed at improving patient experience and quality of care.
- Participate in daily team meetings with healthcare providers.
- Serve as an advocate for patient experience by maintaining a positive demeanor during interactions with patients, visitors, and colleagues.
- Communicate professionally with all patients, visitors, and colleagues, prioritizing customer service while ensuring confidentiality of patient information in accordance with privacy policies.
Additional Responsibilities:
The following tasks are considered secondary to the primary responsibilities listed above:
- Perform various administrative duties as needed.
- May require some travel; a valid driver's license is necessary if traveling.
- Participate in on-call responsibilities as required. Other duties as assigned.
Qualifications:
Minimum Required Qualifications:
- Completion of a Licensed Practical Nurse (LPN) or Registered Nurse (RN) educational program.
- Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
- AHA CPR/BLS certification required within 30 days of hire.
This role necessitates occasional travel to multiple sites.
Preferred Qualifications:
- 1 to 2 years of nursing experience in a primary or specialty care setting.
- Prior experience in triage, emergency care, cardiac care, intensive care, or other related nursing specialties.
