Position Details
LOCATION: Lancaster, PA
HOURS: Full-time 1.0 FTE (40 hours/week); Monday-Friday; 8:30am-5pm; no weekends; no holidays
Sign-on bonus $2500 (pre-established criteria apply)
KEY RESPONSIBILITIES: Qualified candidates must possess the ability (with or without reasonable accommodation) to perform the following tasks:
- Offers clinical guidance via phone/electronic communication based on clinical assessments and/or triage protocols in compliance with regulatory standards and best practices. Collaborates with healthcare providers as needed.
- Delivers patient-specific education covering preventive care, chronic disease management, acute diagnoses, medications, and other areas critical to patient outcomes.
- Prepares patients for appointments by utilizing appropriate rooming standards.
- Administers injections and medications as directed by the healthcare provider and in accordance with departmental protocols, regulatory standards, and best practices.
- Assists healthcare providers with various procedures, minor surgeries, and examinations as assigned.
- Authorizes prescription refills as per provider instructions.
- Supports test tracking and monitors overdue results.
- Removes sutures and changes dressings on wounds as necessary.
- Ensures examination rooms are sufficiently stocked and sanitized after each patient visit. Maintains and cleans medical instruments and equipment following each procedure.
- Collaborates with providers and patients in reviewing and updating care plans and evaluates patient progress. Follows up with patients between visits as necessary.
- Engages in proactive outreach for patients needing preventive and chronic disease services.
- Facilitates care coordination among specialists and transitions of care to gather all necessary patient information.
- Assists in coordinating referrals, education, and community resources.
- Actively participates in quality improvement initiatives to enhance overall patient experience and care quality.
- Participates in daily briefings with the healthcare provider.
- Acts as a patient experience advocate by maintaining a positive demeanor during interactions with patients, visitors, and colleagues. Communicates professionally with all patients, visitors, and colleagues while prioritizing customer service.
- Handles all patient information confidentially in accordance with privacy and confidentiality policies and procedures.
- Performs various administrative duties as needed.
- May require some regional travel - valid driver's license is necessary if traveling.
- Participates in on-call responsibilities for this role as required.
- Other duties as assigned.
- Completion of a LPN or RN educational program.
- Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN), issued by the PA State Board of Nursing.
- AHA CPR/BLS certification required within 30 days of hire.
- One (1) to Two (2) years of nursing experience in a primary or specialty care setting.
- Prior experience in triage, emergency room, cardiac care, intensive care, or other related nursing specialties.
