Job Responsibilities

LOCATION: Lancaster, PA

HOURS: Full-time 0.8 FTE (64 hours/2 weeks); Monday- Friday; varied hours 7:30am-5:00pm; no holidays or weekends.

The Practice Nurse may be required to rotate between multiple facilities to ensure adequate coverage during staff absences.

KEY DUTIES: Qualified candidates must possess the capability (with or without reasonable accommodation) to execute the following responsibilities:
  • Deliver clinical guidance via phone or electronic communication based on clinical assessments and triage protocols, adhering to regulatory standards.
  • Provide patient-specific education on preventive care, chronic disease management, acute conditions, medications, and other relevant topics.
  • Prepare patients for appointments using established rooming standards.
  • Administer prescribed injections and medications in compliance with departmental protocols and regulatory standards.
  • Assist providers with various procedures, minor surgeries, and examinations as directed.
  • Conduct Point of Care Testing, including hearing, flu, urine, and strep tests.
  • Collect and prepare blood or other laboratory specimens for analysis.
  • Authorize prescription refills as per provider instructions.
  • Support test tracking and monitor overdue results.
  • Perform suture removal and dressing changes as required.
  • Ensure exam rooms are adequately stocked and sanitized after each patient visit. Maintain and clean medical instruments and equipment post-procedure.
  • Assist providers and patients in reviewing and updating care plans, assessing patient progress, and following up as necessary.
  • Engage in proactive outreach for patients needing preventive and chronic disease services.
  • Facilitate care coordination among specialists and transitions of care to gather all necessary patient information.
  • Assist in coordinating referrals, education, and community resources.
  • Actively participate in initiatives aimed at improving patient experience and care quality.
  • Engage in daily huddles with the provider.
  • Serve as a patient experience advocate, maintaining professionalism in all interactions with patients, visitors, and colleagues.
  • Handle patient information confidentially in accordance with privacy policies.
ADDITIONAL RESPONSIBILITIES: The following tasks are considered secondary to the primary duties:
  • Perform various front office tasks as needed.
  • Regional travel may be required; a valid driver's license is necessary.
  • Participate in on-call responsibilities as needed.
  • Other duties as assigned.
MINIMUM QUALIFICATIONS:
  • Completion of a LPN or RN educational program.
  • Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) issued by the PA State Board of Nursing.
  • AHA CPR/BLS certification required within 30 days of hire.
  • This role necessitates occasional regional travel; ability to travel to multiple sites is essential.
PREFERRED QUALIFICATIONS:
  • One (1) to Two (2) years of nursing experience in a primary or specialty care setting.
  • Prior experience in triage, emergency room, cardiac care, intensive care, or related specialty nursing.
