We're seeking a skilled Nurse Practitioner or Physician Assistant to join our HouseCalls team in Yakima, WA. In this role, you'll provide annual clinical assessments to patients in the comfort of their homes, helping identify and reduce health risks and coordinating appropriate follow-up care.

About the Role

This is an excellent opportunity to achieve work-life balance with flexible scheduling options. You'll have the ability to flex your time over a 7-day work week, with no on-call, weekends, or holidays required.

Main Responsibilities:

  • Conduct in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as appropriate.
  • Identify diagnoses to inform care management and active medical management.
  • Communicate findings with PCPs to inform potential gaps in care.
  • Intervene in urgent and emergent situations as needed.
  • Educate members on disease processes, medication, and compliance.

Requirements

To be successful in this role, you'll need:

  • An active NP or PA license or ability to obtain by start date, with unencumbered licensure free from any open disciplinary actions.
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or ability to obtain national certification and/or NP or PA license in state of assignment by start date.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs and various dwelling conditions.
  • Reliable transportation for home visit assessments.
