Care Manager

At Ohio's Hospice, we're seeking a dedicated and compassionate Care Manager to join our team. As a Care Manager, you will play a vital role in developing and implementing comprehensive care plans that cater to the unique needs of our patients and their families.
Key Responsibilities:
- Assess patient needs and develop personalized care plans
- Coordinate with healthcare providers and multidisciplinary teams to ensure seamless care delivery
- Facilitate communication between patients, families, and healthcare professionals
- Monitor patient progress and adjust care plans as needed
Requirements:
- Active Registered Nurse license in the State of Ohio
- Associate of Science in Nursing (ASN) or Bachelor of Science in Nursing (BSN) degree
- Three to Five years of clinical experience in Case Management, Care Coordination, Discharge Planning, Medicaid, and/or Medicare managed care
- CM certification preferred
