Comprehensive Cancer Care Specialist

About the Role:
We are seeking a highly skilled Comprehensive Cancer Care Specialist to join our team at Thyme Care. As a key member of our care delivery model, you will play a pivotal role in transforming the cancer care experience for our patients and their loved ones.
Key Responsibilities:
* Conduct comprehensive case management for members navigating cancer and associated comorbidities
* Collaborate closely with the nursing team to review charting, develop care plans, and provide clinical oversight
* Facilitate weekly case conferences to optimize patient care plans
* Provide clinical input and serve as an escalation point for patient care within designated pods
Requirements:
* A Nurse Practitioner with relevant experience and a compact unrestricted Registered Nurse (RN) license in a compact state
* A minimum of 3 years of experience in adult oncology
* Possessing certifications such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) is a plus
