Job Overview

We are seeking a highly skilled Behavioral Health Care Coordinator to join our team at WellSpace Health. This role is responsible for providing high-quality care and support to patients in our THRIVE Program, which aims to provide mental health treatment to justice-involved adults in Sacramento County.

Estimated Salary: $32.45 - $42.12/hr

Key Responsibilities
  • Provide medical assistance and support to patients in the THRIVE Program setting
  • Assist patients with follow-up appointments and provide psychoeducation and case management follow-up
  • Monitor and implement Certified Community Behavioral Health Center (CCBHC) goals and Patient-Centered Health Home (PCHH) goals
  • Support the Behavioral Health Psychiatry providers by performing vitals and conducting case management around medication monitoring, behavioral activation, maintenance, and linking patients with resources in the community
Requirements
  • High School Diploma or equivalent required; experience in human-related fields such as social work, psychology, counseling, alcohol or chemical dependency preferred
  • CPR or CPR/AED certification required
  • Bilingual in Spanish preferred; or other languages desirable
  • Demonstrated commitment to providing services for the underserved and sensitive, working with various people from criminal justice-involved low-income populations with diverse educational, lifestyle, ethnic, and cultural origins
