Job Title: Licensed Clinical Social Worker

AvaMed Workforce is seeking a highly skilled Licensed Clinical Social Worker to join our team at a correctional facility in Vacaville, CA.

Job Summary:

We are looking for a dedicated and compassionate Licensed Clinical Social Worker to provide mental health services to inmates/wards at our correctional facility. The ideal candidate will have a minimum of 1 year of experience and be able to work in a fast-paced environment.

Responsibilities:
  • Provide mental health evaluations, including initial assessments and ongoing treatment planning
  • Conduct clinically sound suicide risk evaluations and develop treatment plans to ensure patient safety
  • Meet with mentally ill patient-inmates/youths for crisis intervention, group psychotherapy, and face-to-face interviews
  • Develop and implement treatment plans that reflect the standard of practice
  • Consult with supervisors, treatment team members, and CDCR institutional health care personnel as needed
  • Document all care provided to inmates, including face-to-face contacts and Interdisciplinary Treatment Team (IOTTs) meetings
  • Monitor inmate progress using CDCR Mental Health approved documentation formatting for progress notes
  • Apply community standard of practices for non-medical treatment modalities and ethical standards
  • Perform mental status exams (MSE) in face-to-face clinical interviews
  • Examine prior mental health records, institutional chromos, probation reports, court records, and other available documents to assist in the formulation of patient diagnosis and aid in the development of treatment plans
  • Conduct mental health education and develop additional resources for mentally ill and sex offender patient-inmates/youths
  • Assist patient-inmates/youths in applying for entitlement benefits (SSI), community programs, and services
  • Facilitate patient-inmates/youths transition between various settings within the institution
  • Perform pre-release functions for releasing patient-inmates/youths, to include patient education, while facilitating transition to county mental health and community mental health treatment programs
  • Perform medical social functions as needed, particularly in CTC setting
  • Participate in IDTT meetings
  • Compile and analyze data from audits pertaining to the mental health program requirements
  • Provide information (i.e. appointment documentation, no-shows, etc.) to support staff for data entry
  • Provide written reports and consults with the Board of Prison Hearings and other CDCR staff
  • Provide relatives/caregivers with information concerning patients in person, by phone, or by correspondence as approved by the supervisor and only after receiving the patient-inmate/wards' signed release of information for that specific individual
  • Develop a therapeutic relationship with the patient-inmates, families, and caretakers, as assigned by the supervisor, in order to enhance effective treatment delivery
  • Represent the Department at formal and informal settings regarding mental health matters related to clinical social work
  • Protect and maintain safety and security of property, including documentation of unusual occurrences and inventory control of psychological testing material, reference material, computers, and other equipment
  • Maintain order and supervise the conduct of persons committed to the correctional facility in order to prevent escapes or injury by these persons to themselves or others
  • Give input and help to implement new programs for the treatment, training, or rehabilitation of patient-inmates/youths
  • Comprehend and adhere to CA and CDCR law, regulations, policies, and procedures regarding the patient's confidentiality, written and electronic medical record documentation, and the release of patient records
  • When conditions are suspected, adhere to mandated reporting requirements pertaining to staff members delivering mental health services, such as reported child abuse, elder abuse, danger to self or other; Tarasoff reporting and other confidentiality mandates
