Job Summary:

We are looking for a highly skilled and compassionate Licensed Clinical Social Worker to join our team. As a Licensed Clinical Social Worker, you will be responsible for conducting counseling sessions with patients experiencing chronic pain, developing treatment plans, and acting as the primary point of contact for patients and healthcare professionals.

Responsibilities:

  • Conduct counseling sessions with patients experiencing chronic pain to help them manage their symptoms and improve their quality of life.
  • Develop treatment plans that are tailored to the individual needs of each patient.
  • Act as the primary point of contact for patients and healthcare professionals to ensure seamless communication and coordination of care.
  • Understand a patient's support needs and interaction preferences to deliver a tailored patient experience.
  • Provide participants with confidential, non-medical counseling services to help them cope with their mental health challenges.
  • Assess the participant's immediate risk and bio psychosocial needs to provide appropriate support and interventions.
  • Deliver educational support to identified patients and caregivers to help them understand their condition and manage their symptoms.
  • Maintain required records on patients and update progress notes as required to ensure clinical chart contains necessary information.

Requirements:

  • Master's degree in Social Work from an accredited institution.
  • Valid and active California state license to practice as a Licensed Clinical Social Worker (LCSW).
  • Minimum of 2 years of clinical experience to the table, with a focus on providing psychotherapy services.
  • Knowledge of solution-based psychotherapy interventions and techniques.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a team environment.
