Job Summary

Turning Point Community Programs is seeking a highly motivated and compassionate Assessment Coordinator I to join our team. As an Assessment Coordinator I, you will play a critical role in ensuring that clients receive high-quality, individualized services that meet their unique needs.

Key Responsibilities
  • Conduct comprehensive assessments of clients to identify their strengths, needs, and goals.
  • Develop and implement individualized treatment plans that address clients' mental health needs.
  • Provide on-going consultation to the treatment team and other staff to support the delivery of services that are driven by the client's plan and supportive of the client's goals.
  • Complete all required paperwork, including annual member paperwork, in a timely manner.
  • Drive on program business using personal vehicle for the purposes of outreach and engagement, assessments, medication outreach, transporting clients to appointments/activities, community/county meetings/trainings.
  • Provide prompt intervention in the event of a crisis both to stop the crisis and to notify, when indicated, persons and agencies necessary for the resolution of the crisis situation.
  • Participate in off-site trainings and provide in-service training for all staff.
Requirements
  • Education, Training and Experience: A typical way of obtaining the knowledge, skills and abilities outlined above is through graduation from a four-year college or university with a bachelor's degree in psychology, Social Work or related field, and a minimum of four years' experience as a provider of mental health services.
  • Licenses; Certificates; Special Requirements: California driver's license & current vehicle insurance/registration, Reliable means of transportation capable of passing vehicle safety inspection if more than five years old excluding all modes of two-wheeled transport inclusive of bicycles, mopeds and motorcycles.

M-F 8:30 a.m- 5:00 p.m.
