Job Title: Telehealth Tech/LPN

Valor Healthcare is seeking a dedicated Telehealth Technician/LPN to provide high-quality patient care and support in our Community Based Outpatient Clinic (CBOC) in Ithaca, NY.

Job Summary:

We are looking for a passionate and experienced Telehealth Technician/LPN to join our team. As a Telehealth Technician/LPN, you will provide prescribed medical treatment and personal care services to patients with diseases and injuries seeking treatment in the clinic, as directed by physician or mid-level provider.

Key Responsibilities:
  • Actively assist with patient care, physical examinations, and ancillary tests via telehealth modalities, including Clinical Video Teleconferencing (CVT), Care Coordination Store and Forward (CCSF), and others.
  • Collaborate with the core PACT Team (Primary Care Provider, RN, and Medical Assistant) and expanded PACT Team to develop patient care plans.
  • Perform annual and routine nursing assessments, preventive health services, and administer medications as appropriate or ordered by a medical provider.
  • Operate electrocardiograph (EKG) and other diagnostic equipment to administer routine diagnostic tests and treatments.
  • Collect lab specimens, clean treatment rooms, and ensure proper clinic stop codes/identifiers are used for each modality.
  • Input patient information into VISTA/CPRS, complete clinical reminders, and assist with front desk duties.
  • Provide patient and family health education, maintain confidentiality of veterans' data, and comply with all VA and company training requirements.
Requirements:
  • Diploma or certificate of an accredited program of practical or vocational nursing as approved by the state.
  • Minimum one-year experience as an LPN/LVN in a related primary care or ambulatory care setting; government healthcare environment preferred.
  • Must be credentialed and remain in good standing through the Veterans Health Administration (VA).
  • Current certification in Basic Life Support and ACLS as specified by individual VAMC contract.
  • Proficiency in written and spoken English and strong computer skills, including EMR experience.
