Compensation and Benefits:

Location:
This role will support hospitals in the Nashville area.
Work Schedule:
Monday to Friday, from 8 AM to 5 PM, with occasional weekend duties.
Qualifications:
- A Bachelor’s degree in marketing, business administration, or a related field is preferred. However, a suitable combination of experience and training may be accepted in lieu of formal education.
- A valid nursing license in the state of practice is advantageous.
- Ability to make independent decisions when necessary.
- Strong interpersonal skills to interact tactfully with patients, families, visitors, agency staff, and the public.
- Familiarity with quality assessment and assurance protocols.
- A valid driver’s license and proof of insurance are required.

Key Responsibilities:
This role is pivotal in collaborating with the Hospice Liaison and Admissions teams to enhance public understanding of hospice services and facilitate patient admissions to meet organizational goals.
The Care Transition Specialist will serve as the primary contact for hospitals and healthcare facilities regarding patient referrals, actively seeking networking opportunities to educate and foster relationships with both new and existing referral sources.
Additionally, this position will be responsible for assessing and identifying suitable care and services for patients and families, in alignment with the attending physician’s directives and the established care plan, while adhering to company policies and procedures.
