Job Opportunity

We are seeking a skilled Geriatric Care Specialist to join our team at Care Connect. As a Senior Healthcare Provider, you will work closely with a board-certified geriatrician to deliver high-quality care to older adults in Assisted Living Facilities and Independent Senior Living Apartments throughout Tennessee.

Key Responsibilities:

  • Provide on-site visits to patients in their homes or facilities
  • Document patient interactions and medical information
  • Collaborate with the geriatrician to develop and implement care plans

Requirements:

  • Tennessee medical license and active DEA number
  • Malpractice insurance provided
  • Experience in geriatric care preferred but not required
