Primary Care Provider

Infinite Medical P.C, a nationwide network of advanced practice providers and specialty clinicians, is seeking a compassionate and skilled Primary Care Provider to join our team. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality, proactive care to residents in skilled nursing and long-term care facilities.
Responsibilities:
- Provide comprehensive healthcare services and coordinate patient care
- Work closely with a team of healthcare professionals to ensure seamless coordination of care
- Assess, diagnose, and treat residents' physical and mental well-being
- Conduct thorough patient assessments, including medical history reviews and physical examinations
- Order and interpret diagnostic tests
- Develop accurate diagnoses and treatment plans
- Prescribe medications and administer treatments
- Provide counseling to residents and their families
- Collaborate with the interdisciplinary team for seamless coordination of care
- Manage chronic conditions and monitor progress
- Make appropriate adjustments to care plans
- Commitment to person-centered care
- Create a safe and nurturing environment for elderly residents in the nursing home
Requirements:
- Nurse Practitioner or Physician Assistant License
- DEA License
- 6 months experience in long-term care

Salary Range:
$120,000.00 - $170,000.00
